When a family finds that a loved one can no longer manage daily tasks or medical care without assistance, Columbine Poudre Home Care, LLC can provide a range of health services. Some of the many vital medical services provided include IV treatment, wound care, post surgical care, physical/occupational/speech therapy, and assistance when recovering from illness or injury. All services provided follow a written plan of treatment established and reviewed by a physician, and access to an on-call Registered Nurse 24 hours a day.

Centre Avenue Health & Rehabilitation Facility provides in-patient therapy allowing residents to regain strength and stamina after a short-term hospitalization or decline in health status. Front Range Therapy provides comprehensive outpatient physical, occupational and speech therapy services.

Columbine Health Systems boasts four skilled nursing facilities: Columbine West Health & Rehab Facility, Lemay Avenue Health & Rehab Facility, North Shore Health & Rehab Facility, and Centre Avenue Health & Rehab Facility. These facilities provide seniors with the highest level of care outside of a hospital. Services include 24-hour skilled nursing care; occupational, physical, respiratory and speech-language therapy; secure memory units; group and individual activities; and transportation to/from medical appointments.

Independent Living: Independent Living, often referred to as Retirement Communities, Congregate Living or Senior Apartments, are designed specifically for independent senior adults who want to enjoy a lifestyle filled with recreational, educational and social activities with other seniors. These communities are designed for seniors who are able to live on their own, but desire the security and conveniences of community living. Some communities offer an enriched lifestyle with organized social and recreational programs as a part of everyday activities, while others provide housing with only a minimal amount of amenities or services.

Assisted living: A type of long-term care facility for elderly or disabled people who are able to get around on their own but who may need help with some activities of daily living, or simply prefer the convenience of having their meals in a central cafeteria and having nursing staff on call.

Nursing Homes: Nursing Homes are designed for seniors who are in need of 24-hour nursing care. Nursing Facilities provide many of the same residential components of other senior care options including room and board, personal care, protection supervision, and may offer other types of therapy. Their onsite medical staff sets them apart from other types of senior housing. Nursing care is provided by registered nurses (RN), licensed practical nurses (LPN), and nurses aides at all hours of the day.
