Home Care When a family finds that a loved one can no longer manage daily tasks or medical care without assistance, Columbine Poudre Home Care, LLC can provide a range of health services. Some of the many vital medical services provided include IV treatment, wound care, post surgical care, physical/occupational/speech therapy, and assistance when recovering from illness or injury. All services provided follow a written plan of treatment established and reviewed by a physician, and access to an on-call Registered Nurse 24 hours a day.

Rehabilitation Centre Avenue Health & Rehabilitation Facility provides in-patient therapy allowing residents to regain strength and stamina after a short-term hospitalization or decline in health status. Front Range Therapy provides comprehensive outpatient physical, occupational and speech therapy services.

Nursing Homes Columbine Health Systems boasts four skilled nursing facilities: Columbine West Health & Rehab Facility, Lemay Avenue Health & Rehab Facility, North Shore Health & Rehab Facility, and Centre Avenue Health & Rehab Facility. These facilities provide seniors with the highest level of care outside of a hospital.
