Social Worker

Masters Level Social Worker (M.S.W.) with experience in health care. Provides services to resolve social or emotional problems that are or are expected to be an impediment to the effective treatment of the client’s medical condition or rate of recovery. Responsibilities include assessment of social and emotional factors affecting the patient, the family/caregiver system, and/or their environment, and the impact of those factors on the medical condition, plan of care, or rate of recovery; counseling for long-range planning and decision-making, education about community resources (and how to access), with assistance in referral process as needed. short-term therapy, crisis intervention to decrease potential for neglect/abuse, decrease suicide potential, increase access to community resources, strengthen fragile support system, and reduce any other high-risk indicator(s) endangering patients physical or mental health.
