Social Services Director
The Social Services Director serves as a resident rights advocate in all manner of speaking. Responsibilities include communicating the needs of residents to the appropriate persons; planning and/or facilitating annual in-services for residents regarding their rights and for facility personnel regarding rights of residents; facilitates care conferences; attends resident council meetings, upholds confidentiality of all information provided by residents, their families, or facility personnel; recruits and provides direction and supervision to department interns; provides information to the Office Manager as applicable regarding admits, discharges, room holds, level of care, and room/unit changes, and day program participation; partners with the Quality Assurance department for the ongoing assessment of medical and psychosocial issues of residents, and determines plan for follow-through regarding contacts and documentation; ensures appropriateness of placement, level of care, and unit; participates in facility meetings as required; documents with consistency and accuracy specific goals, objectives, and progress information for each resident as needed regarding social services issues; and ensures Board and Care Plan is complete and reviewed annually or as needed.
Must have at least a bachelor's degree in the human service field. A Master's degree is preferred. Must have, as a minimum, one year of direct experience working with people with Alzheimer's disease and related memory disorders.
