RN Clinical Coordinator - Assisted Living

Will direct LPNs and care providers and ensure employee adherence to state/facility/company policies.  Ongoing monitoring of residents’ health status and timely physician notification of change in status.  Validation/clarification of physician orders; assist the facility Admissions Director in all admission processes to ensure manageability of potential resident health status and behavior; develop and monitor systems for the effective coordination of disease management for facility residents; develop timely communication systems for reporting information to residents, their representatives, physicians, and others as appropriate; develop quality program to capture and diagnose events to insure continuous quality improvement of resident care.

Nursing degree from an accredited college or university and possess a clear, unencumbered license from the State of Colorado as a Registered Nurse.  Experience working with the geriatric population strongly preferred, particularly with dementia and diabetes.   Experience in quality management and system development and monitoring.  Must display leadership by using effective interpersonal skills.
