Nursing Assistant Enrollment Checklist
Step 1: The prospective student will apply for admission to the program by submitting a completed application to Columbine Health Systems Human Resources Department.
Step 2: The prospective student must obtain a criminal background check and submit to Columbine Health Systems Human Resources Department prior to being considered for placement in a program.
Step 3: The prospective student must obtain a TB skin test and/or Chest X-ray and submit results to Columbine Health Systems Human Resources Department prior to being considered for placement in a program.
The Larimer County Health Department offers TB Skin tests for a minimal fee and has offices in the following locations:

Step 4: The prospective student must pay tuition in full prior to the start of class to guarantee their spot in the program. Payment will be accepted at Columbine Health Systems Human Resources Department, 802 West Drake Road, Suite 145, Fort Collins, Colorado 80526. Reimbursement of fees is as follows:
- Cancellation from the program 7 or more business days before the start of the class will result in full tuition reimbursement minus a $50.00 administrative processing fee.
- Cancellation within 6 business days of the start of the class will result in full tuition reimbursement minus a $75.00 cancellation fee and a $50.00 administrative processing fee.
- Beginning with the first day of class, no reimbursement of funds will be given, regardless of reason for class termination.
Step 5: The student will ensure they have the proper attire for clinical days or attendance at clinicals will not be allowed. Proper attire includes:
- Close-toed shoes
- Forest-green scrubs (one set provided)
- Gait belt (provided)
- Name tag (provided)
