Admission Information
Move-in day can be stressful for residents and their families. Planning ahead can make the transition much easier. Prior to admission, a number of forms must be completed. Please contact our Social Services Director to make an appointment to complete admission paperwork prior to admission.
PLEASE PREPARED TO PROVIDE THE FOLLOWING INFORMATION:
• Full Name
• Mother’s maiden name
• Place of Birth
• Social Security Number
• List of current medications
• Medical care provider names (i.e. attending physician and/or specialists, and dentist)
• Responsible party information (i.e. emergency contacts, phone numbers, and addresses)
• All insurance cards including:
- Medicare card
- Primary insurance card
- Supplemental insurance card
- Medicare D card
• Advanced Directives:
- Living will
- Durable Medical Power of Attorney
- Durable Financial Power of Attorney
- Guardianship / Conservator
- Do Not Resuscitate order (DNR) if applicable
Rooms at New Mercer Commons are unfurnished to allow the resident the opportunity to create their own personal, home-like atmosphere. We recommend residents bring a bed, recliner chair, nightstand, chest of drawers, television, and telephone if desired. Other personal items to bring include clothing, toiletries, bed linens, artwork, and personal pictures. A more detailed list of items to bring is provided in the Resident and Family Handbook.
